Emergency Contact and Pickup Form Please list all emergency contacts and those who are able to pick up your child from camp. NOTE: Anyone picking a child up from camp must have a Photo ID!
Medical History Form Please return this form with your child's most recent immunization record. If you or your child's doctor would like to fax this to us, our Fax is (845)229-6851
Code of Conduct Please review this behavior agreement with your child(ren) before attending camp.
Photo/Video release and Requests Please choose whether or not we can take your child's photo at camp. You may also list any notes or requests, and we will do our best to accommodate you.
